Pulling up to your DOT physical without the right paperwork means rescheduling, losing a day’s pay, and possibly an out-of-service violation if your card expires. This checklist covers everything you need to bring so you get certified in one visit.
Required Documents
- Photo ID (driver’s license or passport)
- Current DOT medical card if you have one
- List of all current medications with dosages
- Eyeglasses or contact lenses if you wear them (bring them, don’t skip them)
- Hearing aids if you use them
If You Have Diabetes
- Most recent A1C lab result (within last 3 months)
- Glucose log if insulin-dependent
- Letter from your endocrinologist or primary care doctor confirming stable management
If You Have High Blood Pressure
- List of BP medications
- Recent BP readings if available (home log helps)
- FMCSA allows up to 140/90 for a 1-year card, 140-159/90-99 for 3 months
If You Have Sleep Apnea
- Sleep study results
- CPAP compliance report (most machines auto-track and your DME provider can print this)
- 4+ hours/night compliance for at least 70% of nights typically required
If You Have a Heart Condition
- Recent EKG (within last year for some conditions)
- Echocardiogram if applicable
- Cardiology clearance letter
- Stress test results if available
If You’ve Had Recent Surgery
- Surgical release or clearance letter
- Records showing post-op stability
What Happens at the Exam
The DOT physical at Garcia Family Medicine typically takes 30-45 minutes. Dr. Tess (FMCSA certified) checks vision, hearing, blood pressure, urine for protein/sugar, range of motion, reflexes, and reviews your medical history. If everything looks good, you walk out with a new 2-year medical card (or 1-year if you have certain conditions).
