Chronic pain is real, debilitating, and one of the most common reasons patients see their primary care doctor. Opioids carry serious addiction and overdose risk. The good news: modern pain medicine has many evidence-based alternatives that work, often better than opioids for long-term management. Here are seven we use at Garcia Family Medicine.
1. Non-Opioid Prescription Medications
The first line of pharmacological pain management is usually non-opioid prescriptions. Depending on the pain type, this can include NSAIDs (meloxicam, celecoxib), neuropathic pain medications (gabapentin, pregabalin, duloxetine), muscle relaxants (cyclobenzaprine, tizanidine), or topical treatments (lidocaine patches, diclofenac gel). These work — sometimes dramatically — without addiction risk.
2. Trigger Point Injections
For musculoskeletal pain with localized “knots” or trigger points, a quick in-office injection of local anesthetic (sometimes combined with corticosteroid) can release the muscle and provide significant relief. Dr. Tess performs trigger point injections regularly for back, neck, and shoulder pain.
3. Joint Injections
Corticosteroid injections directly into painful joints (knees, shoulders, hips) can provide weeks to months of relief for osteoarthritis or inflammatory joint conditions. Hyaluronic acid injections are another option for knee osteoarthritis.
4. Physical Medicine and Rehabilitation
Physical therapy is often more effective than medication for chronic pain — especially back pain, neck pain, and joint pain. A good PT program addresses the root mechanical causes of pain rather than just masking symptoms. We coordinate referrals to trusted physical therapists in the Blue Springs and Kansas City area.
5. Mind-Body Approaches
Chronic pain has a substantial neurological component that responds to mind-body interventions. Mindfulness-based stress reduction, cognitive behavioral therapy for chronic pain (CBT-CP), and biofeedback all have solid evidence. These aren’t “all in your head” — they retrain the nervous system to respond differently to pain signals.
6. Weight Management
For weight-bearing joint pain, weight loss is one of the most effective interventions available. Each pound lost reduces about 4 pounds of force on the knees. A 20-30 pound weight loss often resolves significant joint pain without any other intervention. 7. Lifestyle and Sleep Optimization
Poor sleep amplifies pain perception. Sedentary behavior makes joint and back pain worse. Smoking impairs healing. Addressing these foundational factors — improving sleep hygiene, building gradual exercise tolerance, quitting smoking — often produces results no medication can match.
