One of the most common questions we hear from new patients considering Direct Primary Care: “Can I pay my membership with my HSA or FSA?” Short answer: yes, in nearly all cases. Here’s the full breakdown.
What HSA and FSA Are
A Health Savings Account (HSA) is a tax-advantaged savings account available to people enrolled in a high-deductible health plan (HDHP). You contribute pre-tax dollars, the balance rolls over year to year, and you can use it for qualified medical expenses.
A Flexible Spending Account (FSA) is similar but offered through your employer. You contribute pre-tax dollars to it from your paycheck and use it for medical expenses. Unlike HSAs, FSAs are typically “use it or lose it” by the end of the plan year.
Are DPC Memberships HSA/FSA Eligible?
The IRS allows HSA and FSA funds to be used for “qualified medical expenses.” DPC monthly membership fees generally qualify because they pay for medical services. Most plan administrators accept DPC payments without issue.
That said, some plans have specific rules. Before signing up, contact your HSA or FSA administrator and ask: “Does my plan cover Direct Primary Care membership fees?” Get the answer in writing if possible.
The Tax Savings
Using HSA/FSA funds for your DPC membership effectively reduces the cost by your marginal tax rate. If you’re in the 22% federal bracket plus Missouri state tax (about 5%), your $99/month membership costs you about $72/month in real dollars.
For a family membership at $225/month, that’s a real cost of about $164/month after tax savings — a $700+/year savings vs paying with post-tax dollars.
What Else Can You Pay For?
Beyond your DPC membership, you can typically use HSA/FSA funds for:

How to Pay
At Garcia Family Medicine, we accept HSA and FSA cards directly for membership payments. If your card isn’t accepted (some plans require manual reimbursement), pay with a regular credit card and submit the receipt to your plan administrator for reimbursement.
What Doesn’t Qualify
Most “lifestyle” services don’t qualify, even at a DPC practice — for example, cosmetic procedures or non-medical wellness coaching may not be HSA-eligible. When in doubt, get the qualification confirmed by your administrator in writing.
