How They Work

Tirzepatide (brand names Mounjaro, Zepbound) is a dual GLP-1/GIP receptor agonist. It mimics both GLP-1 and GIP, another gut hormone. The dual action appears to produce greater weight loss for most patients.
Efficacy Comparison
In the SURMOUNT-5 trial directly comparing tirzepatide and semaglutide:
- Tirzepatide: average 20.2% body weight reduction
- Semaglutide: average 13.7% body weight reduction
- Both significantly outperformed placebo
For a 250 lb starting weight, that’s the difference between losing about 50 lbs (tirzepatide) and 34 lbs (semaglutide).
Side Effects
Both medications share the same common side effects, mostly gastrointestinal: nausea, fatigue, constipation, decreased appetite, occasional diarrhea or vomiting. These tend to be worst during dose escalation and improve with time.
Tirzepatide trials show slightly higher rates of GI side effects, particularly at higher doses, though most patients tolerate both well with proper dose escalation.
Dosing
Both are weekly injections (subcutaneous, like an EpiPen). Standard escalation:
- Semaglutide: Start 0.25mg weekly, escalate to 2.4mg over 16+ weeks
- Tirzepatide: Start 2.5mg weekly, escalate to 15mg over 20+ weeks
