GFM Community Connections: Small Business Affiliate Program

This Affiliate Program Agreement ("Agreement") is entered into by and between Garcia Family Medicine ("GFM"), located at [Insert Address], and the participating business entity ("Affiliate") in the GFM Community Connections: Small Business Affiliate Program ("Program").

By joining this Program, the Affiliate agrees to the following terms and conditions:

1. Program Overview

The GFM Community Connections Program is designed to strengthen relationships between local businesses and Garcia Family Medicine by rewarding businesses for referring new patients while providing exclusive incentives to their customers.

2. Eligibility & Enrollment

• Open to registered businesses and organizations that align with Garcia Family Medicine’s mission.
• GFM reserves the right to approve or reject applications at its discretion.
• Affiliates must ethically promote GFM’s services and display marketing materials.
• Affiliates cannot offer medical advice on behalf of GFM.

3. Referral Tracking & Incentives

• Each Affiliate is assigned a unique QR code for tracking referrals.
• Customers must scan the QR code and complete a Spin-the-Wheel Entry Form upon check-in.
• Successful referrals qualify the customer for the Spin-the-Wheel Giveaway.
• Prizes include discount vouchers and one free month of membership (valued at $200).

4. Affiliate Benefits

5. Responsibilities of the Affiliate

• Must actively promote GFM’s services and display marketing materials.
• Cannot engage in deceptive or misleading marketing practices.
• Must ensure that referred customers understand how to enter the giveaway.

6. Program Terms & Termination

• Agreement remains in effect until terminated by either party with 30 days’ written notice.
• GFM may terminate an Affiliate’s participation for unethical marketing or rule violations.
• Upon termination, the Affiliate must remove all GFM marketing materials.

7. Confidentiality & Data Protection

• Non-public business information shared between parties must remain confidential.
• Patient data will not be collected; referral data is used strictly for tracking and promotions.

8. Limitation of Liability

GFM is not liable for indirect or incidental damages resulting from Affiliate participation. No guarantees are made regarding referral numbers or marketing exposure.

9. Governing Law & Dispute Resolution
