Giveaway Terms and Conditions

1. Sponsor

This giveaway (“Giveaway”) is sponsored by Garcia Family Medicine (“Sponsor”), located at 801 NW St. Mary Drive, Suite 209, Blue Springs, MO 64014.

2. Eligibility

  • Open to legal residents of the United States who are 18 years of age or older at the time of entry.
  • Employees, officers, and directors of the Sponsor, its affiliates, and their immediate family members are not eligible to participate.
  • Void where prohibited by law.

3. Giveaway Period

The Giveaway begins on February 1, 2025, at 12:00 AM CST and ends on February 13, 2025, at 11:59 PM CST (“Giveaway Period”). Entries submitted outside the Giveaway Period will not be accepted.

4. How to Enter

  • Participants must complete the online entry form available at https://www.garciafamilymedicine.care/valintines-day-gift-card-giveaway.
  • Required information includes full name and a valid email address.
  • Limit one (1) entry per person. Multiple entries by the same person will be disqualified.
  • No purchase is necessary to enter or win.
  • Entrants must check a box to agree to the terms and conditions of the Giveaway and acknowledge how their information will be used.
  • If opting into marketing emails, participants must check a separate box agreeing to receive marketing communications.

5. Prize

  • One (1) winner will receive a $250 gift card.
  • The prize is non-transferable and cannot be exchanged for cash or any other goods or services.
  • The Sponsor reserves the right to substitute the prize with an item of equal or greater value if the advertised prize becomes unavailable.

6. Winner Selection and Notification

  • The winner will be selected randomly using Random.org to generate a random number between 1 and the total number of unique participants.
  • Each eligible entry will be assigned a unique number, and the corresponding number generated by Random.org will determine the winner.
  • The winner will be notified via the email address provided during entry. The winner must respond within 72 hours to claim the prize. Failure to respond within this period will result in forfeiture of the prize, and another winner will be selected.
  • The Sponsor is not responsible for incorrect or invalid email addresses or other contact information provided by entrants.

7. Use of Personal Information

By entering, participants consent to the Sponsor’s collection, use, and storage of their personal information solely for the purposes of administering this Giveaway. Personal information will not be shared with third parties or used for marketing purposes unless participants opt in to receive such communications. Participants may request the deletion of their personal information at any time by contacting the Sponsor.

8. Publicity

By accepting the prize, the winner agrees to allow the Sponsor to use their name, likeness, and entry details for promotional purposes without additional compensation, unless prohibited by law.

9. General Conditions

The Sponsor reserves the right to cancel, suspend, or modify the Giveaway at any time without prior notice due to unforeseen circumstances or technical failures. Participants who attempt to tamper with the entry process or engage in fraudulent activities will be disqualified. The Sponsor’s decisions are final and binding in all matters related to the Giveaway.

10. Limitation of Liability

The Sponsor is not responsible for any technical malfunctions, lost entries, or other errors that may occur during the entry process. By participating, entrants release the Sponsor from any liability arising from participation in the Giveaway or acceptance, receipt, or use of the prize.

11. Governing Law

The Giveaway is governed by the laws of the State of Missouri, without regard to its conflict of law principles.
